The parent and minor must present their government-issued photo IDs, The only alternative ID accepted for a minor is their birth certificate. Minors under 16 can only get earlobe piercings with parental consent, while those aged 16 or 17 can get most other piercings (except nipple and surface piercings). This form is to be filled out prior to receiving a piercing
