Kindly follow these steps [as of March 31 2025]

1. Approach a gate with an ID Terminal.
Security personnel will verify your identity and enrollment status, and you will be issued with a temporary campus pass good for one day.

2. Secure a notarized Affidavit of Loss.
This will protect you from the risk of any unauthorized use of your lost ID. You may get a copy of an affidavit of loss at a legitimate notary public office, create an affidavit of loss, or grab a template off Google and edit the necessary details needed. Have the affidavit notarized by a legitimate notary public. 

3. Request for a NEW ID card at The Hub.
Bring the affidavit and proceed to the Enrollment Services Hub at 2F Henry Sy Sr. Hall. Pay the ID fee and expect a new ID card in 3-5 days.

What should I do if I lost my ID?

When can I apply for a DL certificate? 			

Yes, processing fees depend on which document is requested and the time the registrar needs to prepare them. Refer to this link for the fees [only accessible from 8AM Monday till 12PM Saturday]: https://ecom.dlsu.edu.ph/apps/ord/document/policies.asp

Is there any processing fee for requesting a document? How much?			

Concerns may be accommodated by sending an email to hub@dlsu.edu.ph or by contacting the Enrollment Services Hub via The CONCiERGE Support Portal.

How can I request for document if the site of DLSU is down?			

If my request took more than 20-30 working days to process, who and where can I follow up my request?			

In-person collection of documents is strictly by appointment only. The appointment link will be sent once the documents are available. The office of the Enrollment Services Hub is on the 2nd floor of Henry Sy Sr. Hall Building.

If I opted to personally get the document I requested in campus, where can I find it?			

No. In case of declared office work suspension or changes in campus entry policies, the appointment is automatically deemed canceled and the client must book a new appointment.			

If my scheduled appointment date is declared as holiday or the work and classes are suspended, will I still go to campus?			

A separate email containing a tracking number will be sent once the documents are endorsed to DLSU’s courier partner. Once documents are released to the courier, the delivery status can be tracked via the courier website.

How can I track the status of my document?			

Effective Term 3 of the Academic Year 2024-2025, the following are the official payment channels:

1. Online Bills Payment through BDO, BPI, RCBC, and UNIONBANK.

2. Online through DLSU’s Online Payment Gateway (OPG) using:
VISA and Mastercard credit cards and debit cards
Dragonpay (Gcash, Maya; please see the other available payment channels at the dropdown menu of the Dragonpay payment page; subject to payment partner fees)

3. Over-the-counter (OTC) payment at any branch of BDO, RCBC, and UNIONBANK.

What is the mode of payment in requesting for a document?

How can I request for TOR?			

The processing period is different and distinct from the delivery period and is subject to the courier’s lead times.
