Medicaid

MEDICARE - Many versions and combinations
Pt has unknown Medicare Supplement, or does not know card number, or 65+ with no information.
•	1-800-633-4227 "1-800-MEDICARE" 
•	"Verify benefits“ - "no“ - "no"
•	Will have a brief hold to talk to a rep and then must provide information with the client present to get account information. 
•	Medicare and has insurance information.

Call Assigned Clinic or Supplement Plan support line to find out Primary Care Physician information and connect to case management services.
•	Medicare.gov is a great resource if they have an account setup.

Medicaid Supplement Plan
•	Can get confusing, See Medicaid

Medicare Plans SHOULD offer case management assistance, nurse line, etc. If having difficulty, refer to Neighborhood Center or other community partners.

      •	Might need to update financial and living situation to be eligible for VA Clinic Services.

MAKE SURE IT IS A MARKETPLACE PLAN FIRST!!!

Many can look like similar: Medicare, Medicaid, VA/Tricare Supplement, etc.

Once confirmed is is a Marketplace Plan AND they are eligible for MAP POS-Application, discuss cancelling and enrolling in MAP.

    Reason: "Transitioning to a local health program“

Ineligible for MAP POS-Application?
